A motorist reported in July 2014 that another driver had threatened him and later gone to his work and demanded to know where he lived. Nine days later the complainant left a message on the officer’s mobile with information about the other man’s car. However he never heard from the officer again.

The Police Ombudsman found the officer took no further steps to investigate despite being asked to do so by his supervisors.
